H1N1 Vaccine Consent Form
Below is a link for the form that must be filled out prior to coming to one of the H1N1 clinics. Please download the form and bring the completed form to the H1N1 clinics. All practitioners attending an H1N1 clinic must register with Melissa to ensure that there is enough vaccine at the clinic.

The Department of Health has notified the regional EMS councils that OSHA requires each service to offer the H1N1 vaccine to all its members. If a member refuses the vaccination, below is a link to a sample refusal form.

Dr. Brian Risavi, DO, EMMCO West Regional Medical Director, RMAC, and the EMMCO West Board of Directors have released a resource guideline to assist medical command physicians determine the care and transport of STEMI and Stroke patients in the EMMCO West region.

Photo Id's & Signatures can now be imprinted onto PA Department of Health Id Cards. Beginning September 15, 2008, EMS personnel can come to the EMMCO West Office between 8 - 4:30 PM Mon - Friday to have their photo id and signatures imprinted onto a new certification card. No appointment is necessary. EMMCO West will have the photo id and signature processing at different regional events. Personnel will need a copy of their EMS certification, current CPR, and a form of other photo id (drivers license, passport, school id, work id).
